Told You Do Not Have Enough Bone for an Implant?

It is one of the commonest things patients hear, and it rarely means implants are impossible. It usually means an extra step.

An implant needs bone around it on every side to hold it and to stay healthy. When a tooth is removed, the bone that used to hold it begins to shrink — most noticeably in the first year, and continuing slowly afterwards. Wait long enough and there may not be enough width or height left for an implant without rebuilding the site first.

The main procedures

  • Socket preservation. Graft material placed into the socket at the time of extraction, to limit the shrinkage before it happens. Small, quick, and by far the cheapest way to deal with this problem, because it prevents it.
  • Ridge augmentation. Rebuilding width or height where bone has already been lost, usually with graft material and a membrane, left to mature for several months.
  • Sinus lift. In the upper back jaw, the sinus sits above the roots. If there is not enough height beneath it, the sinus floor is gently raised and graft placed underneath. Common, well-established, and often done at the same time as placing the implant when enough bone remains to hold it steady.
  • Guided bone regeneration. Smaller grafting alongside implant placement, to fill a minor defect around the post.

What gets grafted

Usually processed graft material from a sterile bank source or a synthetic mineral, which acts as a scaffold your own bone grows into and gradually replaces. Sometimes a small amount of your own bone is used. The material is not permanent in itself — the point is the bone your body builds through it.

What it adds

Mainly time. Socket preservation adds little. A larger ridge augmentation or sinus lift typically needs four to nine months to mature before the implant goes in, which can turn a six-month implant plan into a year. It also adds cost, and that cost should be in the quote from the beginning rather than appearing later.

Recovery

Swelling for several days, manageable with anti-inflammatories and cold packs. Soft diet for about a week. After a sinus lift specifically: do not blow your nose for a couple of weeks, and sneeze with your mouth open, to avoid disturbing the graft.

How to avoid needing one

Decide about the gap before the tooth comes out. If an implant is likely at any point, ask for socket preservation at the time of extraction. It is a small addition to that appointment and it can save a much larger, longer and more expensive graft later. The patients who need major augmentation are usually those who lost the tooth years ago and are only now considering replacement.

If you have been told implants are impossible, it is worth a second look. Angled implants, shorter implants, or narrower ones can sometimes avoid grafting entirely, and a 3D scan will settle what your case actually allows.
