Dry Socket: How to Tell It From Normal Healing

Extraction pain that fades and then returns worse on day three is the classic pattern. It has a specific cause and a quick fix.

Dry socket — alveolar osteitis — happens when the blood clot in an extraction socket is lost or breaks down before the gum has healed over. The bone and nerve endings underneath are then exposed directly to air, food and saliva. It affects roughly 2 to 5 per cent of routine extractions and considerably more lower wisdom tooth removals.

The pattern that identifies it

Normal extraction pain is worst on day one and improves steadily from there. Dry socket does the opposite. Pain settles for a day or two, then returns sharply on day three or four, often worse than the original.

The other features are distinctive:

  • A deep, throbbing ache that radiates to the ear, eye or temple on that side
  • Pain that ordinary painkillers barely touch
  • An empty-looking socket, sometimes with visible whitish bone instead of a dark clot
  • A foul taste and noticeably bad breath

Importantly, dry socket does not usually cause fever or significant facial swelling. If you have those, it is more likely an infection, which is a different problem.

Why it happens

  • Smoking. By far the largest risk factor, through both suction and reduced blood supply. Smokers have several times the risk.
  • Rinsing, spitting or using a straw in the first 24 hours.
  • Difficult or surgical extractions, particularly lower wisdom teeth.
  • Existing infection around the tooth before removal.
  • Oral contraceptives, which raise the risk somewhat.
  • A previous dry socket, which makes another more likely.

What treatment involves

This is one of the more satisfying things we treat, because relief is usually immediate. The socket is gently irrigated to remove debris, then packed with a medicated dressing containing a soothing agent such as eugenol. Most patients describe the pain dropping away within minutes.

The dressing may need changing every couple of days for up to a week. Antibiotics are usually not required, because dry socket is not primarily an infection — it is exposed bone. Prescribing them routinely is unnecessary.

It heals fully in seven to ten days. There is no lasting damage.

Prevention

Do not smoke for at least 72 hours — ideally longer. Do not rinse, spit or use a straw for the first 24 hours. Keep the mouth clean with gentle salt-water rinses from day two. If you have had a dry socket before, tell us beforehand, so a dressing can be placed at the time of extraction.

Do not wait it out

Dry socket does eventually resolve on its own, but it can mean seven to ten days of severe pain that a five-minute appointment would have ended. There is no reason to endure it.

Frequently asked questions

How do I know if I have dry socket or just normal pain?

Timing is the giveaway. Normal pain improves steadily from day one. Dry socket pain eases, then returns sharply on day three or four, radiates towards the ear, and barely responds to painkillers.

Does dry socket need antibiotics?

Usually not. It is exposed bone rather than an infection. Treatment is irrigation and a medicated dressing, which relieves the pain almost immediately. Antibiotics are added only if infection is also present.

How long does dry socket last?

Seven to ten days untreated. With a dressing, the pain typically stops within minutes and the socket heals over the same period. There is no long-term damage either way.

Can I get dry socket after a normal extraction?

Yes, though it is less common than after surgical or lower wisdom tooth removal. Around 2 to 5 per cent of routine extractions are affected, rising sharply in smokers.
