When a tooth comes out, the gap does not stay a tidy gap. The teeth either side begin to tilt into the space, and the tooth in the opposing jaw — with nothing to bite against — drifts down or up out of its socket. Over years this changes how your teeth meet, which loads other teeth abnormally and makes cleaning harder.
Bone changes too. Jawbone maintains its density because a tooth root stimulates it every time you chew. Remove the root and the bone in that area resorbs, most rapidly in the first year. This matters practically: waiting several years often means a bone graft is needed before an implant becomes possible.
Dental implant
A titanium post placed into the jawbone, which fuses with it, topped with a crown. It is the only option that replaces the root as well as the crown, which is why it is the only one that preserves bone.
For: does not involve the neighbouring teeth at all; feels and functions closest to a natural tooth; longest lifespan, frequently decades. Against: highest upfront cost; needs adequate bone; total treatment takes three to six months while the bone integrates; needs a minor surgical procedure.
Fixed bridge
A crown on each neighbouring tooth with a false tooth joined between, cemented in place.
For: fixed and stable; completed in about two weeks; lower upfront cost; no surgery. Against: the two healthy neighbouring teeth must be cut down permanently to take crowns — irreversible, and a real cost if those teeth were untouched; bone under the gap still resorbs; typically lasts 10 to 15 years.
Removable partial denture
A removable plate carrying one or more false teeth.
For: least expensive; no surgery; no cutting of other teeth; can replace several teeth at once. Against: removable, so it moves slightly; least comfortable; clasps may show; needs cleaning out daily; bone continues to resorb, so it needs relining or remaking over time.
How to choose
If the neighbouring teeth are healthy and untouched, an implant is usually the better long-term decision precisely because a bridge would mean damaging them. If those teeth already have large fillings or crowns, a bridge makes more sense — they need crowning anyway. If several teeth are missing, or if cost is the deciding constraint, a denture is a legitimate answer and not a failure.
Cost per year of service is a fairer comparison than the sticker price. A bridge replaced twice over thirty years can end up costing more than one implant.
What about a back tooth nobody sees?
The common instinct is to leave it. The trouble is that the opposing tooth over-erupts and the neighbours tilt, and ten years later the problem is no longer one gap but a bite that needs substantial work. If a wisdom tooth is the one lost, it usually does not need replacing at all — but any other tooth generally does.
Frequently asked questions
How long can I leave a gap before getting an implant?
Bone loss is fastest in the first six to twelve months. An implant can still be placed years later, but you are more likely to need a bone graft first, which adds cost and time.
Is a dental implant painful?
Placement is done under local anaesthesia and most patients report less discomfort than an extraction. Expect mild soreness for two to three days, managed with ordinary painkillers.
How long do dental implants last?
With good cleaning and regular check-ups, frequently 20 years or more; many last a lifetime. The crown on top may need replacing sooner than the implant itself.
Can I get an implant if I have diabetes?
Usually yes, provided blood sugar is reasonably controlled. Poorly controlled diabetes slows healing and raises failure risk, so we assess this before planning treatment.
