A gap between the upper front teeth is called a midline diastema. In many cultures it is considered attractive and needs no treatment at all. If you want it closed, the sensible first step is establishing why it is there, because that determines which treatment will hold.
The common causes
- Normal growth. Between about seven and twelve, a gap is often a passing stage — sometimes called the ugly duckling phase. The erupting canines push the roots of the front teeth together and the gap usually closes itself. Treating it before the canines arrive is often unnecessary.
- A large labial frenum. The band of tissue running from the inside of the upper lip to the gum can extend down between the teeth and physically block them from meeting.
- Teeth small relative to the jaw. If the jawbone is wide and the teeth are narrow, spacing is simply arithmetic.
- A missing or peg-shaped lateral incisor, which leaves the front teeth spreading into the space.
- Tongue thrust, where the tongue presses forward on swallowing, gradually pushing the teeth apart.
- Gum disease. This is the one that matters most. When bone is lost, teeth drift and splay, and new gaps appear in adults who never had them. A gap that has appeared recently in an adult mouth should be investigated before it is closed.
The options
Composite bonding is the quickest: tooth-coloured material is sculpted onto the sides of the two front teeth in a single visit, with no drilling in most cases. It is reversible, the least expensive option, and typically lasts five to eight years before it needs polishing or replacing. It can pick up staining at the margins over time.
Veneers are thin ceramic facings, more stain-resistant and longer lasting, but usually requiring a small amount of enamel to be removed. That makes them irreversible, which is worth understanding before agreeing to them.
Braces or clear aligners actually move the teeth together rather than making them wider. This is the right answer where the gap is large, where several teeth are involved, or where the bite needs correcting anyway. It takes months rather than an hour, but nothing is added to or removed from the teeth.
A frenectomy — releasing the tissue band — may be needed alongside orthodontics where a large frenum is the cause. Done alone, the gap usually reopens.
Retention is the part people skip
A gap closed by orthodontics will reopen without a retainer. For midline diastemas specifically, a thin wire bonded behind the front teeth is usually recommended long term, because this is one of the most relapse-prone corrections in orthodontics. Budget for it from the start.
Frequently asked questions
Will the gap between my child's front teeth close on its own?
Often, yes. A gap at seven or eight frequently closes as the canines erupt and push the front teeth together. If it is still there once the canines are through, it is unlikely to close by itself.
Can a gap be closed without braces?
Yes, with composite bonding in a single visit, or with veneers. Both widen the teeth rather than moving them, which suits small gaps. Larger gaps look better closed orthodontically.
Why has a gap suddenly appeared between my teeth as an adult?
New spacing in an adult mouth commonly signals bone loss from gum disease, which lets teeth drift. It should be assessed before any cosmetic work, because closing the gap does not treat the cause.
Does a gap between the front teeth cause dental problems?
Usually not by itself. It can occasionally affect speech or leave gums exposed to food impaction, but a stable midline gap in a healthy mouth is a cosmetic matter, not a health one.
