A Lump on Your Gum: Dental Abscess, Explained

A gum boil is pus finding a way out. It often stops hurting once it drains, which is exactly why it gets ignored.

An abscess is a walled-off collection of pus produced when bacteria get somewhere the immune system cannot easily reach. In the mouth there are two common kinds, and telling them apart changes the treatment entirely.

The two types

A periapical abscess starts inside the tooth. Decay reaches the pulp, the nerve dies, and infection travels out of the root tip into the bone. The tooth is usually tender to bite on and feels slightly raised. Treatment is root canal or extraction.

A periodontal abscess starts in the gum pocket beside the tooth, usually where gum disease has already created a deep space, or where something like a popcorn husk or a seed has been forced down. The tooth is often still alive. Treatment is drainage and deep cleaning of the pocket.

Why the boil bursts and the problem stays

Pus under pressure hurts. When it finds a channel to the surface — the small pimple on the gum, called a sinus tract — the pressure drops and the pain largely stops. Patients reasonably conclude it has healed.

It has not. The drainage channel simply means the infection now has somewhere to go. The dead nerve is still dead, the bacteria are still multiplying, and the bone around the root tip is slowly being destroyed. A discharging sinus can persist for years while the underlying damage grows, which is why these are often found on X-rays taken for something else entirely.

Antibiotics are not a cure

This is worth being clear about. Antibiotics reduce the surrounding infection and can settle swelling, but they cannot reach inside a tooth with a dead nerve, because there is no blood supply left to carry them there. The source has to be dealt with mechanically — by cleaning out the canal, draining the pocket, or removing the tooth. Repeated courses of antibiotics without treating the source simply breeds resistance and delays the inevitable.

What is genuinely urgent

Go to a dentist the same day, or to a hospital if none is available, for any of these:

  • Facial swelling that is spreading, or swelling that closes the eye
  • Swelling under the jaw or into the neck, or a raised floor of the mouth
  • Difficulty swallowing, breathing or opening the mouth
  • Fever with dental pain, or feeling generally unwell

Dental infections that spread into the floor of the mouth can compromise the airway. That is rare, but it is the reason nobody should sit out facial swelling over a weekend.

Until you are seen

Rinse with warm salt water several times a day, take an anti-inflammatory if it is safe for you, sleep propped up, and do not apply heat to a swollen face. Never try to lance a swelling yourself.

Frequently asked questions

My gum boil burst and the pain stopped. Do I still need treatment?

Yes. The pain stopped because the pressure was released, not because the infection cleared. The dead nerve or infected pocket is still there and the bone around the root continues to be damaged.

Will antibiotics clear a dental abscess?

They can settle swelling and make you feel better, but they cannot cure it. A tooth with a dead nerve has no blood supply to carry antibiotics inside. The source must be treated by root canal, drainage or extraction.

Is a dental abscess an emergency?

It becomes one with facial swelling, fever, difficulty swallowing or breathing, or swelling spreading towards the eye or neck. Those need same-day care, or hospital assessment outside clinic hours.

Can an abscessed tooth be saved?

Often yes. Root canal treatment removes the infected pulp and seals the canal, and the bone around the root usually heals over the following months. Whether it can be saved depends on how much tooth structure remains.
