Advertising says implants are permanent. That is a simplification worth unpacking, because two different components wear on very different timescales.
The implant itself
The titanium post fused into the bone is the durable part. Long-term studies consistently report survival above 90 per cent at ten years, and a large proportion still functioning at fifteen to twenty. Many last a lifetime. Titanium does not decay, which removes the commonest reason natural teeth are lost.
The crown on top
This is the part that wears out. Expect roughly ten to fifteen years from the crown before it may need replacing, and the small screw holding it occasionally needs retightening in between. That is normal maintenance, not failure, and it is worth budgeting for.
What actually causes implants to fail
- Smoking. The single largest controllable risk, raising failure rates substantially through reduced blood supply and impaired healing.
- Peri-implantitis — gum disease around the implant. Plaque around an implant causes bone loss just as it does around a tooth, and it is harder to treat because the implant surface is not smooth like a root.
- Uncontrolled diabetes, which impairs healing and gum health.
- Grinding, which overloads the implant. An implant has no shock-absorbing ligament, so it cannot cushion force the way a natural tooth does. A night guard protects it.
- Poor position, placed where it cannot be cleaned properly.
Notice how many of those are within your control. Implant longevity is less about the product than about smoking, cleaning and review.
How to protect it
Clean around it daily — a water flosser or interdental brushes reach where floss cannot pass under the crown. Keep six-monthly reviews, because peri-implantitis is painless early and obvious late. Wear a night guard if you grind. Do not smoke. Keep your implant card so future components can be matched.
The comparison worth making
An implant is excellent, and it is still second to your own tooth. A natural tooth has a ligament that senses pressure and absorbs shock; an implant is fused rigidly to bone and has neither. That is why, when a tooth is borderline, attempting to save it with root canal treatment is usually worth trying first. Implants are the best replacement available — they are not an upgrade on a healthy tooth.
