Jaw Clicking, Locking and Pain: What TMJ Problems Actually Mean

A clicking jaw on its own is usually harmless. Clicking with pain, or a jaw that locks, is a different matter.

The temporomandibular joint sits just in front of each ear and is the most-used joint in the body — it moves every time you speak, chew, swallow or yawn. Between the ball and socket is a small cartilage disc that slides with the movement. Most jaw problems come down to that disc slipping out of position, or to the muscles around the joint being overworked.

The reassuring part

A jaw that clicks but does not hurt, does not lock, and opens normally needs no treatment at all. Painless clicking is extremely common and, on its own, is not evidence of damage or of a problem that will worsen. Treating it is usually unnecessary.

When it does need attention

  • Pain in the joint, the cheek muscles or in front of the ear.
  • Locking — the jaw catching when opening, or not closing properly.
  • Limited opening, less than roughly three of your own fingers stacked vertically.
  • Headaches on waking, particularly at the temples.
  • Earache with normal hearing and a normal ear examination.
  • A bite that suddenly feels different.

What causes it

Most cases are muscular rather than structural. Clenching and grinding — often at night, often stress-driven — keep the jaw muscles contracted for hours, and muscles held under load ache exactly like any overworked muscle. Other contributors include chewing on one side because the other side hurts, habitual gum chewing, nail biting, a missing back tooth that unbalances the bite, an injury to the jaw, and arthritis in the joint itself.

What genuinely helps

Treatment starts conservatively, because most cases settle without anything invasive:

  • Rest the joint. Soft food for two weeks, no gum, cut food into small pieces, and support your chin when you yawn.
  • Moist heat over the muscles for 15 minutes twice a day for muscular pain; cold packs if the joint itself is acutely inflamed.
  • Anti-inflammatories for a short course, if they are safe for you.
  • A night guard, which is the mainstay where grinding is involved. It does not stop the grinding, but it protects the teeth and lets the muscles settle.
  • Jaw exercises and posture work, which have good evidence behind them.
  • Restoring a missing tooth where the bite has collapsed on one side.

Irreversible treatment — grinding down teeth to adjust the bite, or surgery — is a last resort, and should never be the first thing offered. The overwhelming majority of jaw pain improves with conservative management.

When to be seen quickly

A jaw that has locked open, a jaw that cannot be opened more than a couple of centimetres, or jaw pain following a blow to the face all need seeing promptly. Jaw pain with chest pain, breathlessness or pain radiating down the left arm is not a dental problem — that is a medical emergency.

Can a night guard cure TMJ pain?

It rarely cures the underlying habit, but it protects the teeth from grinding wear and allows overworked jaw muscles to settle, which relieves the pain in a large proportion of cases.

Why does my jaw hurt more in the morning?

That pattern strongly suggests night-time clenching or grinding. The muscles have been contracting for hours while you sleep, so they are at their most fatigued when you wake.
