Dentistry involves judgement, and competent dentists genuinely differ on borderline cases. A second opinion is not an accusation. It is ordinary prudence on anything expensive or irreversible, and it is standard practice in every other branch of medicine.
Situations clearly worth a second look
- You have been told to extract a tooth, particularly a front tooth or one that is not loose. Extraction is irreversible, and an endodontist may see a way to save what a general plan writes off.
- A large plan appeared at a first visit — a full-mouth proposal for someone who came in about one painful tooth.
- Multiple crowns or veneers on healthy teeth for cosmetic reasons. This removes tooth structure permanently.
- A quote far above or far below others, in either direction.
- A root canal has failed and extraction is proposed. Re-treatment often works.
- You do not understand why the treatment is needed, and asking did not clarify it.
- Something feels rushed. Pressure to decide today is not a clinical argument.
When it is probably unnecessary
Routine fillings, cleanings, a straightforward extraction of a badly broken tooth, or an obvious abscess needing urgent treatment. Delaying genuine urgency to shop around is its own risk.
What to bring
Bring your X-rays — you are entitled to copies, and a second opinion based on the same images is far more useful, besides sparing you another dose of radiation. Bring the written treatment plan and quote, and a note of any medicines you take.
One suggestion: consider not saying what the first plan was until after the examination. You will get an independent view rather than a reaction to somebody else's.
Questions worth asking
- What happens if I do nothing for six months?
- Is there a less invasive option, even if it lasts less long?
- Which parts are urgent and which can wait?
- What would you do if this were your own tooth?
When two opinions differ
They often will, and both can be defensible. Ask each to explain the reasoning rather than the price. The plan that removes less tooth, keeps options open for later, and comes with a clear explanation of the alternatives is usually the sounder one. If the difference is stark, a third opinion is not excessive for major work.

As an endodontist-led practice we are biased in one specific direction, and it is worth stating: we would rather try to save a tooth than replace it.
